How Little Spoon Supports collects, uses and protects personal information.

Little Spoon Supports respects your privacy. This policy explains the kinds of personal information we may collect, why we collect it, how it may be used or shared, and how you can ask questions or request access or correction.

Who we are

Little Spoon Supports is an independent, unregistered NDIS provider based in the Ipswich region. Privacy enquiries can be sent to admin@littlespoonsupports.com.au.

Information we may collect

Depending on how you interact with us, we may collect information such as:

  • your name, preferred name, contact details and relationship to the person being referred;
  • basic referral details, including age, suburb, funding type, requested supports and availability;
  • information you choose to provide about disability, support needs, communication, safety, health or other circumstances relevant to deciding whether we can safely provide support;
  • complaints, feedback and correspondence;
  • service, scheduling, invoicing and payment information if you become a client; and
  • basic technical information generated when you use the website, such as browser or device information, where collected by our hosting or form providers.

Please only provide information that is relevant to your referral, enquiry or feedback. You do not need to send complete NDIS plans, detailed medical records or identity documents through the website unless we specifically ask for them through an appropriate process.

Why we collect information

We may use personal information to respond to enquiries, assess referrals, decide whether our service is a suitable and safe fit, arrange and provide supports, communicate with clients and authorised representatives, manage complaints or incidents, meet record-keeping and legal obligations, and administer our business.

Information about another person

If you provide information about another person, you should have authority or a reasonable basis to do so. Where appropriate, we may confirm consent or authority before progressing a referral or sharing information.

How website forms are handled

Our referral and complaints forms are embedded on this website and processed using Jotform. Although the form appears within the Little Spoon Supports website, the information entered into it is processed through Jotform's systems. Our website is hosted through Netlify and our business email is managed through Microsoft 365.

If you become a client, we may also use business systems such as ShiftCare and Xero for service administration, records, scheduling and invoicing, where relevant.

When we may share information

We do not sell personal information. We may share information where reasonably necessary to provide or administer services, with your consent or authority, with service providers that help us operate our business, or where disclosure is required or authorised by law. We aim to share only what is reasonably necessary for the purpose.

Storage and security

We use reasonable administrative and technical safeguards to protect information, including access controls and multi-factor authentication on relevant business accounts. No online system is completely risk-free, so please avoid sending unnecessary sensitive information by ordinary email or website form.

Retention

We keep records for as long as they are reasonably needed for the purpose they were collected, to provide services, resolve complaints, meet funding or record-keeping requirements, or comply with applicable law. Information that is no longer required will be securely deleted or de-identified where appropriate.

Access and correction

You can ask us what personal information we hold about you, request access to it, or ask us to correct information that is inaccurate, out of date or incomplete. In some circumstances we may need to verify your identity before responding.

Changes to this policy

We may update this policy when our services, systems or legal obligations change. The current version will be published on this page with the date it was last updated.
